Description: Join us for an hour of learning as speakers address the importance of screening people for nutrition risk.  Learn how Alberta Health Services has launched a web-based screening tool for use by the older adult in the community setting.

In addition, gain knowledge about barriers to successful self-management of nutrition risk after hospital discharge and strategies for patient-informed solutions.
