Request Samples

Direct to Patient Samples:

  • Samples are provided for tolerance trials only. Products should be tube fed. Oral nutrition supplements may not be requested unless there is a tolerance concern.
  • Delivery to patient may take approximately 5-7 business days once the request is processed.
  • Samples are limited to 1 order per patient.
  • Please be sure to double check the patient's mailing address.
  • By submitting your patient’s information and ordering samples, you are certifying that you have determined that this product is medically appropriate for your patient to sample, that you have explained this to your patient, and that you have obtained your patient’s consent to have samples of our products shipped directly to their home address.

Request Sample

Through the Nestlé Medical Hub, you can receive samples and patient education materials, delivered to your office. These product samples and materials are provided to you at no charge by Nestlé Health Science to test patient tolerance. (Need to include copy that this is just a request, not an actual order placed)

These products are not to be resold and no medical insurer, including private insurers or any government healthcare program, may be billed for the sample product. We do not allow sampling to urgent care facilities. Nestlé Health Science reserves the right to limit quantities. Eligibility conditioned upon the intended individual and while supplies last. This sampling program is subject to change or cancellation at any time. Please allow 6-8 weeks for delivery.
